Pay Application Fee: $50
The application which has 2 parts and then the optional information form section. Have copies of your transcript available to assist you in completing the information. After you complete a section, print out a copy for your records, then hit the "submit" button, and then move on to the next section. You must enter your name and social security number on each form.
