Application Process - Berlin, Germany

Specific documentation may vary slightly from country-to-country based upon the education, regulation and licensing systems. The application process is in two steps:

Step 1:

• Complete the Non-Degree Application form online in order to register for course work.

Step 2:

• Completed Full Application form online, due no later than January 4, 2013

  • Copy of your original academic credential – staatlich geprüfter Augenoptiker
    • This academic credential must also be accompanied by a notarized English translation of the document.
  • Copy of your professional license to practice optometry (or associated official documentation of country regulations)
    • This professional license must also be accompanied by a notarized English translation of the document.
  • One letter of recommendation from a professional organization, teacher or colleague.
