Application Process - Berlin, Germany
Specific documentation may vary slightly from country-to-country based upon the education, regulation and licensing systems. The application process is in two steps:
Step 1:
- Complete the Non-Degree Application online form in order to register for course work.
Step 2:
- Completed Full Application online form, due no later than January 4, 2013

- Copy of your original academic credential – BSc Augenoptic, BSc Augenoptik/Optometrie or BSc Optometrie
- This academic credential must also be accompanied by a notarized English translation of the document.
- Copy of your professional license to practice optometry (or associated official documentation of country regulations)
- This professional license must also be accompanied by a notarized English translation of the document.
- One letter of recommendation from a professional organization, teacher or colleague.
- Copy of your original academic credential – BSc Augenoptic, BSc Augenoptik/Optometrie or BSc Optometrie
- Non-refundable application fee of $150 USD - Pay Online

Please note: In order to qualify for final admission into the degree program, an applicant must complete the full application process and submit all required credentials (see Step 2).
